Cook County Local Rule 13.3.1 provides that parties to divorce cases involving property division, child support, maintenance or certain other issues  issues (whether the issue arises before or after the divorce) must complete financial disclosure statements. The parties are required to list their income, expenses and assets on the financial disclosure statement as initial discovery in the case and then they may engage in additional discovery (subpoenas, document production, interrogatories, depositions).  You may download a blank financial statement by clicking the PDF link below.
